New Patient Information

We are currently accepting new patients for all of our services, if you are interested in being seen in our practice, please call our office at 336-282-1251 to schedule an initial evaluation. 

New Patients who have already been registered are encouraged to complete our New Patient Packet from our Forms page. Please note that if the new patient is a minor, we will need a Consent to Treat Minor Form completed as well. ( Please read it carefully, sign or initial where indicated, and either email (gvcpfax@gmail.com) or fax (336-282-1251) prior to your initial appointment. Please also include a copy of your insurance card (if applicable).
